Cognitive-Behavioral Conjoint Therapy for PTSD: Improving PTSD and Enhancing Relationships

Candice M. Monson, Ph.D., Ryerson University, Toronto, ON, Canada.

Posttraumatic Stress Disorder (PTSD) is associated with a myriad of intimate relationship problems, and family functioning has been associated with the efficacy of individual evidence-based PTSD treatment. This presentation will provide an overview of Cognitive-Behavioral Conjoint Therapy (CBCT) for PTSD, a therapy designed to simultaneously decrease PTSD and other trauma-related symptoms, improve relationship functioning, and enhance the well-being of significant others. CBCT for PTSD is a 15-session, manualized therapy comprised of 3 phases. The first phase focuses on psychoeducation about traumatic stress-related problems and an interpersonally-oriented rationale for treatment, as well as conflict management strategies. Phase 2 has a simultaneous focus on improving communication and decreasing experiential and behavioral avoidance. Phase 3 involves specific dyadic cognitive strategies aimed at problematic trauma appraisals and trauma-relevant beliefs held by the couple. Results of uncontrolled and controlled trials support the efficacy of the therapy with a range of couples. Case examples will be used to illustrate the delivery of key interventions. Clinical considerations in incorporating intimate others in PTSD assessment treatment will be discussed (e.g., trauma disclosure; dual traumatization; intimate aggression).
